Do you have a pre-existing / pre-disposing medical condition, serious injury or have you suffered from decompression illness?
There is a requirement to declare any pre-existing / pre-disposing medical conditions, serious injuries or previous decompression illness. This requirement is not onerous and is there to help guide our members to discuss any potential problems with a diving doctor, if appropriate. In some cases, the diver may be required to obtain a Fitness to Dive certificate in accordance with the guidelines of the South Pacific Underwater Medicine Society (SPUMS). However, in most cases to date, no further action is necessary. It is also providing us with some valuable information about the types of conditions that people are diving with.

Please note your application cannot be processed until this form has been received.

Are you a non-National of Australia?
If you are not an Australian National you need to reside in Australia for at least 3 months to be eligible for DAN Membership only. However, this period is extended to 12 months if you wish to take out the optional Dive Injury Insurance as well.
Should you make a claim, you may be required to provide evidence of your long-term residency in Australia for your claim to be approved.
Do you intend to travel or live outside Australia for longer than 3 months?
If you are an Australian National intending to travel or live outside of Australia for longer than 3 months during your insurance period you should contact the DAN Asia-Pacific office to discuss your options as you may need to take out additional coverage as the optional dive injury insurance will not cover you for the extended period. It is essential that you are registered with DAN AP at the place where you are actually residing for extended periods.
